In this tutorial I used the tube "Witch 14" from

Unless noted, I used a drop shadow of vertical 0, horizontal 5, opacity
85, blur 10.
Open a new transparent canvas 750 by 750 pixels and fill with white.
Paste the mask, resize 22%, select all, float and defloat, make a new layer,
paste paper 10 into the selection, deselect and delete the original black mask
layer.
Paste element 43, resize 35%, open your Pick tool, set the mode to Scale,
hold down the Shift key and pull one of the bottom side nodes out a little,
take the hold off the Shift key and push the centre back node down a little,
sharpen. Your element should now look something like my screenshot below.
Position the element and apply a shadow of vertical and horizontal 0, opacity
100, blur 15.
Paste element 158, resize 40%, mirror and position below element 43.
Paste element 157 on the top, resize 50% and move into place.
Paste element 130, resize 40%, position and apply the shadow (remember to
change the shadow settings).

Check your layers and make any adjustments, then crop your tag, add your
