I used png mask 378 from Mizteeques Digital Designs.

I applied a drop shadow of vertical 0, horizontal 5, opacity 75, blur 10.

Open a new transparent canvas 750 by 850 pixels and fill with white.

Paste frame 6 and resize 50%, select the interior with your magic wand, expand the selection by 20, make a new layer below the frame and paste paper 17 into the selection, deselect, mirror the paper, apply the shadow to the frame, merge it down onto the paper and position.

Paste element 25, resize 55%, position and apply the shadow.

Paste element 45, resize 40%, position and apply the shadow.

Paste element 27, resize 40%, position below element 45 and apply the shadow.

Paste element 23 on the top, resize 20%, position and apply the shadow.

Paste element 79, resize 25%, position and apply the shadow.

Paste element 83, resize 15%, position and apply the shadow.

Paste element 126, resize 15%, position and apply the shadow.

Paste element 94, resize 15%, position and apply the shadow.

Paste element 9, resize 15%, position and apply the shadow.

Paste element 96, resize 15%, position and apply the shadow.

Paste element 72, resize 10%, position and apply the shadow.

Paste element 59, resize 10%, position and apply the shadow.

Paste element 111, resize 8%, position and apply the shadow.

Paste element 87, resize 22%, position and apply the shadow.

Paste tube 5, resize 30%, position and apply the shadow.

Paste element 37, resize 45%, mirror, position below the tube and apply the shadow.

Paste element 34, resize 15%, position and apply the shadow.

Paste element 40, resize 30%, position and apply the shadow.

Paste element 50 on the top, resize 12%, position and apply the shadow.

Paste element 51, resize 12%, position and apply the shadow.

Paste element 104, resize 7%, position and apply the shadow.

Paste element 70, resize 15%, position and apply the shadow.,

Paste element 102, resize 7%, position and apply the shadow.

Paste element 133, resize 7%, position and apply the shadow.

Paste element 97, resize 15%, position and apply the shadow.

Paste element 55, resize 10%, position and apply the shadow.
 
Paste element 64, resize 12%, position and apply the shadow.

Paste element 38 just above the white background, resize 70%, move it down so a bit shows below the tube and bottom group of elements, erase all the part that shows above that bit, we just want a base for those elements, then apply the drop shadow.

Check your layers and make any adjustments.

Open the mask, duplicate it and close the original, select all, float and defloat, make a new layer, paste paper 10 into the selection, deselect, copy the paper and paste above the white background, resize 70% and position and the top, duplicate, flip the copy, move it down so a little shows at the bottom and then erase the bulk of it leaving just that little bit at the bottom, duplicate the first copy again, mirror it and position so some shows on the left.

There are so many lovely elements in this kit, maybe you would like to add some more!  Then when you are done, crop, add your information and export on a transparent background as a png.
