I used a drop shadow of vertical 0, horizontal 5, opacity 75, blur
10.

Open a new transparent canvas 750 by 750 pixels and fill with white.

Paste element 45, duplicate, merge the copy down onto the original and move
up and to the right.

Paste element 19.

Paste element 52, resize 80%, duplicate, position one copy on the right and
one on the left towards the top.

Paste element 126 and position between elements 52.

Paste element 127, mirror and move down and a little to the right.

Paste element 5, position and apply the shadow.

Paste element 2, resize 60% and position on the right, duplicate twice, flip
and mirror one copy and position at the top, mirror the other copy and position
on the left, apply the shadow to each.

Paste element 132, resize 40%, position and apply the shadow.

Paste element 151, resize 80% and mirror, position and apply the shadow.

Paste element 107, resize 50%, position and apply the shadow.

Paste element 103, resize 40% and mirror.   Click anywhere on your canvas
outside of the element with your magic wand to make a selection, invert the
selection and contract it by 10, make a new layer below it and paste paper 9
into the selection, deselect, apply the shadow to the frame, merge it down onto
the paper and move into place.

Paste the tube, resize 30%, position and apply the shadow.

Paste element 10, resize 20%, position below the tube and apply the shadow.

Paste element 29, resize 30%, position and apply the shadow.

Paste element 1, resize 15%, position and apply the shadow.

Paste element 164, resize 40%, position and apply the shadow.

Paste element 3, resize 12%, position and apply the shadow.

Paste element 154, resize 90% and apply the shadow, position below the tube,
draw a rectangular selection around the bottom part of the ribbon just below the
middle bow and hit delete, deselect, right click on your canvas and "paste as a
new image", the cut off piece will paste back, bring this piece above the tube
and join the two sections up.

Paste element 153 below the tube, resize 40%, position and apply the
shadow.

Paste element 134, resize 30%, position and apply the shadow.

Paste element 130, resize 30% and rotate 10 degrees to the left, position and
apply the shadow.

Paste element 80, resize 35%, position and apply the shadow.

Paste element 78, resize 30%, position and apply the shadow.

Paste element 60, resize 15%, position and apply the shadow.

Paste element 136, resize 18%, position and apply the shadow.

Paste element 133, resize 30%, position and apply the shadow.

Paste element 147, resize 60%, position and apply the shadow.

Check your elements position so far then go back to your tube, set your
eraser to size 100 and opacity 10 and run it around that harsh corner bottom
left to blend it in.

Paste element 111, resize 25%, position below the top piece of ribbon and
apply the shadow.

Paste element 62 on the top, resize 35%, position and apply the shadow.

Paste element 139, resize 27%, position and apply the shadow.

Paste element 156, resize 18%, position and apply the shadow.
