0038 and 0048.

Font of choice.

I used a drop shadow of
vertical 0, horizontal 5, opacity 85, blur 10.

Open a new transparent
canvas 750 by 800 and fill with white.

Make a new layer, select
all, paste paper 25 into the selection, deselect, duplicate the paper and close
the top copy off.  Apply mask 0038 to the
opened paper then resize it 80% and mirror it. 
Open the top copy of the paper, apply mask 0048 to it and resize 80%.

Paste element 144,
resize 75%, duplicate and move both copies into place.

Paste element 45,
resize 65% and mirror, position and apply the shadow.

Paste element 150, resize
80%, position and apply the shadow.

Paste element 16,
resize 40%, position and apply the shadow.

Paste element 30, resize
35%, position and apply the shadow.

Paste element 49,
resize 40%, position and apply the shadow.

Paste element 34,
resize 35% and mirror, position and apply the shadow.

Paste frame 6, resize
60%, select the interior with your magic wand and expand the selection by 15,
make a new layer below it and paste paper 25 into the selection, deselect,
apply the shadow to the frame, merge it down onto the paper and move into place.
